Submitting Your Application:

Thanks for choosing to be part of a truly unique and wonderful experience helping children with first time dental care. Below is the list of application documents required to register for a project. The Application Checklist provides a complete list of the forms required to complete the process. Please open and print each document, complete and return to our office with a $700 deposit (which is applied to your project fee).

After You Enroll:

Within two weeks of submitting your application you can expect to receive an acceptance packet which includes information on obtaining a visa, flight options to reach the project site, vaccination recommendations and other details to guide you through the pre-departure process.
